What is home health?

Home health is a medical service prescribed by a physician to help with rehabilitation after an illness, hospital stay or surgery, or to help manage a chronic medical condition.

Medical care is provided through home visits with specialized clinicians until your recovery goals have been met.

Episodes of care typically last between 30 and 60 days but can extend when medically necessary.

How Home Health Works?

Ones we receive an order from your doctor, we will contact you to setup an appointment for a nurse or physical therapist to visit you at home. The clinician will then assess your medical needs, arrange for other specialists to visit, and develop a treatment plan which may include a home safety evaluation, home exercise program, and the coordination of community resources and services.

All home health visits are generally covered 100% by Medicare or private insurance.
